It's not really a mystery, it's clearly a scam, plain and simple.

I would have sent what was owed and cashed out the balance. Let them figure it out.

As this was a 6 hour spinal surgery, it would not surprise me if the hospital+ doctor charges were well over $200,000! Notice how these massive amounts of money move from insurer to provider and the patient is just a token, a marker. The entire medical establishment is one big money laundering scam with massive overcharges for unnecessary procedures. Back surgery does not help and often makes symptoms worse. All of these spinal problems are nutritional and reversible with supplements, diet and physical therapy costing a few hundred dollars at most.

We have a medical monopoly charging outrageous amounts for unnecessary and dangerous procedures because they can! You want this criminality to stop? Stop feeding the monster! Drop your health insurance. Remember, health insurance is to guarantee that the hospital and doctor get paid, not to guarantee that you get cured! Fact: 99% of diseases can be reversed and/or prevented with nutritional supplements and diet. You don't have to live in fear of getting heart disease, cancer, arthritis, rheumatoid disease, autoimmune disease, diabetes, high blood pressure, osteoporosis or Alzheimer disease. All of these are easily preventable. The only legitimate role for modern medicine is in the treatment of acute trauma and surgical emergencies. If you need these done and have no insurance, you can offer the hospital and doctor a cash out-of-pocket settlement and they will gladly accept 20% of the inflated bill. So if you bank the health insurance premia (instead of sending them to a bloated health insurance bureacracy) in a year or two you will have enough cash to handle any emergency. If all of us dropped insurance and starting taking charge of our own health modern healthcare system would collapse, as it should. Stop feeding the healthcare monster and living in fear of sickness - fix yourself and save yourself a lot of money and unnecessary suffering.

Insurance is a form of gambling where you're betting against yourself.

US citizens DESERVE the slavemaster's whip when they don't follow orders (because they are slaves via their own volition).

All that comes to mind, is that it has to be an insurance scam of some kind, I'd be tempted to keep the money and offer to split it with them...
