That's a hard question.

If I were famous and hated I could say that those who hate me belong to the anti-Ash-society (ASS for short). That society may not exist but once the term has been used those who hate me may identify themselves as ASS. Now does it exist?

I voted none of the above, but I actually think that it has some of all of the above... To start of with it was a CIA creation, and CIA has kept OBL alive like Emmanuel Goldstein for our daily 1 minute of hate. Probably it has some disorganized cells outside of control, that believes they are Al Qaeda, but don't really pose any serious threat... 911 was a pure mossad led operation, where they threw in some Al Qaeda disinformation, to point the investigators in the right direction, the investigators were of course mossad as well...
