It's just a phrase describing how individual stupid or crazy turns into group stupid or crazy. It starts with a boss who has, or supports a stupid or crazy idea. Everybody whose salaries or promotions depend on that boss feels obliged to go along and refrain from commenting on how stupid or crazy the idea is. It's not as elegant a description of this process as The Emperor's New Clothes.
