Most of the lighting you see is the positive streamer from the ground which comes up to meet the stepped leader that first emerges from the cloud. Often times the stepped leader is not visible all the way to the ground because it doesn't contain all that much current. The "return stroke" from the ground has the hotter, higher current, which is why you're more likely to see it.

To add to that, if and when you are outside and you feel your hair start to stand up and your skin feels funny, hit the ground or find shelter immediately and make as low of a profile of yourself as you can. It is the ground up feelers that you would be feeling.

I have seen coronas discharge develop on antennas because of this.
