Outside of perforating the ear drum, the little hairs, and the nerves they are connected to begin to break down.

One sign of damage is constant "Ringing" and sometimes hearing what sounds like bass guitar or drum beats that are mixing it up. 24/7/365 and you are the only one that can hear it. Sometimes the sounds are very loud and are hard to ignore, and other times completely not heard at all. And on a side note there is not a thing that can be done, except live with it and keep some real background noise going all the time. So the ringing/phantom noise is not so noticeable.
Plus certain frequency ranges can be confusing to understand or even hear.
