Part I section 3 IRS FORM W-8BEN ... there is a checkbox for UNION STATE CITIZENS on this IRS form. To determine your identification as a NON-RESIDENT ALIEN CLAIMING NON-U.S. PERSON STATUS. To be frank there are too many negatives for me in that section header. I keep trying to interpret that as RESIDENT ALIEN CLAIMING U.S. PERSON STATUS.

I had an entity trying to get me to fill out and sign one of those FORM W-8BEN and instead I filled out and signed one of the pre USA-PATRIOT ACT W-8 forms - the obvious distinction being the 'BEN' stuff being applicable to someone who's a 'beneficiary' (meaning someone who's not a principal, i.e. one who has a beneficial interest rather than ownership).

I suspect a rat. Doubt if that link goes to a real IRS form and probably is one that has been doctored. The concept of a UNION STATE CITIZEN would raise the specter of a CONFEDERATE STATE CITIZEN.

Something IS fishy with that as I just looked up the W-8BEN forms revised in '08, '12, and '14 - NONE of them have those check box options to be found anywhere on the form. Note the form at the link you posted has no revision date on it (found in upper left corner).
