I found this device to work really well and it can be had on feebay for <$60. Works so well I bought one for every automobile in the fleet. Made in the Philippines, not China -

I assume it's a bluetooth connection? And is that a little speaker on the left? Is it loud enough to hear when you're driving up and down the highway?

Great options for older vehicles that may not have BT. Curious to hear about how they work (as MF asks)

Does it transmit incoming to the car radio?

I do have to turn the volume up all the way to hear it consistently but duplex conversation is just fine, those on the other end have not complained in fact when asked they tell me the audio quality is just fine. The only problem I've had is when in close proximity the device will handshake with my iphone when outside the truck or SUV and I won't hear a call when it's linked since the device rings when linked.

Exactly, don't want to be installing new bluetooth capable indash radios on every truck.

No, it's completely standalone.
