It shouldn't cost $700 either. More like $100-200. A radiologist should be able to analyse and write a report on at least 40 CT-scans a day 20 days a month=800/month x$12/CT scan= $9,600 salary after social security tax insurance $7,000/month. Some may argue the Radiologist should earn more than $84,000 a year. I disagree, sure it requires some analytical skill, but as a radiologist you don't deal with people directly, which means the job is fairly stress free and cushy. Sure a few can earn more than $7,000 a month, but that is only because they read more films either working longer hours or working more efficiently or both. Or they have distinguished themselves analytically. Then you need a radiology nurse, to handle the CT scanner He or She'll do about 30 scans in an 8-hour shift=600 images/month x $8/image=$4,800/month which translates to a salary of $4000/ month or $48,000 annually.
So running cost salary for one CT scan should be around $20/scan. Let's assume you have three CT-machines, running 16 hours/day in 2 shifts. =180 scans/day (For that you need 6 radiology nurses and 4.5 Radiologist MDs). Let's assume these machines with computers and software cost $1,000,000 each (they should probably cost less though). You have an investment of $3,000,000 which you write off in 5 years, equals $600,000/year. At 36,000 scans/year that overhead equals $16.67/scan. Then you pay $3000/month in rent for the location=$36,000/year or a dollar/scan. The receptionist booking and charging patients cost another $36,000/year or a dollar a scan. A radiologist is the radiology clinic's boss and expert radiologist, but is only asked about the tricky films, earns $144,000/year or $4/film. Then add another $108,000 for maintenance of machines and office supplies= $3/scan. Anyway you look at it you can run this operation at $50/scan, add a 20% profit for the investor, and a 20% bonus for the employees (in free time/holiday, or Christmas bonus). You are still looking at less than $75 a scan, if you can run a smooth operation like the one I outlined here, and everyone earns a descent salary, and investors are payed off well too (will get back the whole investment in approximately three years, assuming no bank loans were taken out). Machines will probably last at least 10 years with proper maintenance!
Charging $700 would be criminal. $3,700 should carry a death penalty.
The thing is that the majority of imaging in the hospital system is clinically unnecessary. From my experience in Turkey I would say about 80-90% are unnecessary. They are only (or mainly) done to earn money. Probably a bit less in US. A lot less in Sweden (due to socialist health care system, sad thing though due to the same system many necessary are not done either)
